Application form builder reference

This page describes the settings and field types available in the application form builder — the tool you use to choose which questions appear on each level’s sign-up form.

Type What it does
text A single-line text input. Suitable for names, titles, addresses, or short answers.
email An email address field. The main email field is system-locked required and cannot be removed.
checkbox A single true/false checkbox. Use for newsletter opt-ins or consent questions.
select A dropdown with a fixed list of options. The options are defined in Member Database settings and can’t be edited from the form builder.
textarea A multi-line text box for longer answers.
Setting What it does Notes
Label The question or label the applicant sees on the form. Required.
Required When checked, the applicant must answer this field to submit. Email is always required and cannot be unchecked.
Show on form When unchecked, the field is hidden from the applicant but not deleted. You can re-show hidden fields later.
Badge Meaning
Required — always System-locked required field (email only). Cannot be hidden.
Required Currently required per your form configuration.
Optional Not required; applicants can leave it blank.
System core A built-in field (email, first name, last name, phone). Field type cannot be changed.
Level scoped A custom field that only appears on this level’s form. Can be promoted to appear on all levels.
Hidden The field is configured but not shown to applicants.
Setting What it does Notes
Require terms acceptance When checked, applicants must tick an “I accept” checkbox before they can submit. A timestamped audit record is created for each acceptance.
Terms text The text shown above the acceptance checkbox on the form. Up to 2,000 characters. Plain text; links are rendered as-is.

The Embed tab generates a code snippet you can place on your own website so people can apply without leaving your site.

Setting What it does
Theme Color scheme for the embedded widget: Default (your org brand), Light, or Dark.
Allowed websites The domain names where the embed code will be used. Your browser won’t load the widget on any website not listed here. Add each domain where you’ll place the embed code.
